Method
- Preheat oven to 350°F (177°C).
- Combine all ingredients except beef and ketchup. Mix well, then gently fold in beef.
- Shape into a loaf in a baking dish and bake for 30 minutes.
- Spread ketchup on top, if using, and bake for another 25–45 minutes until internal temperature reaches 160°F (71°C).
- Let rest for 10 minutes before slicing and serving.

Notes
- For extra moisture: Add 2 tablespoons of heavy cream or grated zucchini to the mixture.
